How to Do a Self Skin Exam at Home: A Dermatologist’s Guide

Knowing how to do a skin self-exam using the ABCDE method is one of the most valuable things you can do for your long-term health — especially in Central Texas, where year-round sun exposure accelerates the cumulative UV damage that drives melanoma and non-melanoma skin cancers. Self-exams don’t replace annual professional screening with dermoscopy, but they give you the ability to detect changes between appointments and alert your dermatologist before something progresses. This guide covers exactly what to look for, how to look, and what to do with what you find.

Why Self-Exams Matter for Melanoma Detection

You live in your skin every day. You notice your moles when they change — if you’re looking. That observational advantage is real: a significant proportion of melanomas are first detected by patients or their partners, not by clinicians. The challenge is knowing what to look for.

The stakes are high. According to the American Cancer Society, the 5-year relative survival rate for melanoma that is still localized to the skin is greater than 99%. Once it has spread to distant sites, that figure falls to 36%. The window between the two is catching melanoma early — which is what both regular self-exams and annual professional dermoscopic screening are designed to do.

The ABCDE Method

The original ABCD criteria were developed to help patients and clinicians identify the visual features most associated with melanoma in pigmented lesions, and were described in foundational work by Friedman et al. (CA Cancer J Clin, 1985). The “E” for Evolving was added two decades later by Abbasi et al. (JAMA, 2004). Together they have become the standard patient-education framework for mole self-assessment worldwide.

A — Asymmetry: Draw an imaginary line through the center of the mole. In a benign mole, both halves roughly mirror each other. In a concerning lesion, one half looks meaningfully different from the other. Asymmetry is one of the earliest detectable features of melanoma growth.

B — Border: Benign moles have smooth, clearly defined edges. Concerning lesions have irregular, notched, scalloped, or poorly defined borders that appear “eaten away” or blurred at the edges.

C — Color: A single, uniform shade of brown or tan is typical of benign moles. Melanoma often displays multiple colors within the same lesion — various shades of brown, black, tan, red, white, or blue. This color variation reflects different types and depths of pigment deposition and immune reaction.

D — Diameter: Lesions greater than 6 millimeters (roughly the diameter of a pencil eraser) warrant closer evaluation. However, this criterion is the least specific — some melanomas are smaller than 6 mm when they first appear. Don’t dismiss a smaller lesion that meets other criteria.

E — Evolving: This is the most clinically sensitive criterion. Any mole that is changing — in size, shape, color, texture, or elevation — should be evaluated by a dermatologist. Change over time is a stronger signal than any single visual feature. Also watch for new symptoms: itching, tenderness, bleeding, or crusting of a previously stable mole.

Important limitation: The ABCDE rules do not detect all melanomas. Nodular melanoma, in particular, often grows rapidly as a uniform, raised, dark or pink nodule that does not fit the ABCDE pattern well. Any rapidly growing new lesion warrants prompt evaluation regardless of ABCDE status.

The Ugly Duckling Sign

An important supplement to ABCDE is the “Ugly Duckling” sign: a mole that looks distinctly different from your other moles — the one that doesn’t fit the pattern of your personal baseline. Most people’s moles share a family resemblance in size, color, and shape. A mole that stands out as the “odd one out” among your other moles should be evaluated, even if it doesn’t clearly meet ABCDE criteria. In an image-based study, Scope et al. (Arch Dermatol, 2008) found that observers across a wide range of training levels consistently flagged the same “ugly duckling” lesions — supporting it as a sensitive, easy-to-apply screening cue alongside ABCDE.

How to Perform a Full-Body Self-Exam

Perform a methodical full-body exam once monthly, on a consistent date. You need: a well-lit room (natural light is ideal), a full-length mirror, a handheld mirror, and a smartphone for documentation.

Face, ears, and neck: Examine your entire face including the nose, lips, and earlobes. Use the handheld mirror to check behind your ears and the back of your neck.

Scalp: Part your hair in sections and examine your scalp carefully — melanoma can develop here and is often caught late due to hair coverage. A comb helps expose each section.

Arms and hands: Examine front and back of both arms from shoulder to fingertips. Check between every finger and under each fingernail. Subungual melanoma (under the nail) presents as a brown or black streak in the nail and is frequently missed.

Torso: Use the full-length mirror for the front. Use the handheld mirror to check the back, including the midback and lower back — areas that are often overlooked and frequently sun-exposed.

Legs and feet: Examine both legs front and back from the groin to the toes. Check between toes and the soles of the feet. Acral lentiginous melanoma (on the palms, soles, and under nails) is the most common melanoma subtype in patients with darker skin tones and is frequently found on the feet.

Genitals: These areas receive less sun but can develop melanoma. Use the handheld mirror to examine areas you cannot easily see directly.

Documenting Your Moles Over Time

Photographing moles monthly with your smartphone creates a personal baseline that makes subtle evolution obvious. For each photo, note: location (e.g., “left shoulder, 3 cm below the clavicle”), approximate size, date, and any notable features. Comparing photos across months is far more sensitive for detecting change than relying on memory alone.

When to Call Your Dermatologist

  • Any lesion meeting one or more ABCDE criteria
  • A lesion that looks different from all your other moles (Ugly Duckling sign)
  • Any mole that is clearly changing between monthly exams
  • A new lesion that grows over days to weeks (consider nodular melanoma)
  • A sore that bleeds, itches, crusts, or fails to heal after 3–4 weeks
  • A dark streak appearing under a fingernail or toenail

The goal is not to panic about every mole — it is to develop a systematic habit of observation so that genuine changes are caught promptly. Most evaluated lesions are benign. The cost of a biopsy for a benign lesion is minimal compared to the cost of delayed melanoma diagnosis.

The Limits of Self-Exams

Self-exams are powerful but imperfect. You can miss lesions on your back or scalp. Early melanomas can be visually subtle. No untrained eye can reliably distinguish all melanomas from benign nevi. Dermoscopy — a clinical tool that reveals subsurface patterns invisible to the naked eye — substantially improves diagnostic accuracy in trained hands. Annual professional full-body screening with dermoscopy provides a level of sensitivity that self-exams cannot replicate.
